We are staffed with compassionate professionals, ready to assist residents in the transition from hospital to home.
Our highly-skilled nurses provide the best quality care with compassion and grace. Their skills are only half the picture. Each nurse is a gentle and caring person, with a servants heart and genuine concern for each resident.
Village Health Care partners with the CRM physician group. Led by Dr Christopher Morris, CRM is a multidisciplinary team of physicians and nurse practitioners brought on board with one goal in mind—to improve patient care while reducing the frequency of hospital readmissions.

PATIENT CARE MANAGEMENT
The role of our Patient Care Managers is to carefully monitor each patient and develop a care plan that will encourage them to reach their full potential.
Additional assistance is provided by our Social Services Department to establish realistic goals, help with transitional needs, and assist with placement plans upon discharge. Whether you’re recovering from a surgery, injury, or illness, we’re ready to partner with you for a successful recovery!
"I really appreciated not only the clinical care but the organization and forethought that went into every decision and suggestion beginning at admission."
